Transition services are a coordinated set of activities for a student with a disability that promotes a movement from school to school and from school to post-school activities.
Incorporation of transition services into the IEP process must take place by age 16. This includes early identification of, and planning for, the student's post-school goals by defining a course of study.
Transition is based upon the individual student's needs, and takes into account the student's preferences and interests.
Post-school activities include post-secondary education, vocational training, integrated employment, continuing and adult education, adult services, independent living, and/or community participation. If another agency is going to be involved in post-graduation planning (i.e. Vocational Rehab), then a representative from that agency must be part of the transition planning team.
